Understanding 24-Hour and Ongoing Private Nursing
Not everyone requires nursing care around the clock.
Some individuals benefit from longer daytime shifts, overnight assistance, or recurring nursing visits several days each week. Others may require continuous skilled nursing support throughout the day and night.
For clients needing 24-hour coverage, nursing is generally coordinated across multiple independent nurses working separate shifts.
Depending on the requested schedule, arrangements may involve dedicated daytime and nighttime nurses, nurses sharing recurring weekly assignments, or a larger nursing team providing coverage seven days a week.
TLC works with clients and families to understand the nursing hours requested and identify independent nurses whose qualifications, experience, and availability align with those needs.
The goal is to develop a practical, organized nursing arrangement that supports the client’s medical needs while respecting personal preferences, established routines, and the nurses’ professional independence.

When Extended or Around-the-Clock Nursing May Be Helpful
Families explore ongoing private nursing for many different reasons.
A loved one may be recovering from a significant procedure, returning home after hospitalization, living with a progressive medical condition, or requiring skilled nursing assistance throughout the day or night.
Nursing needs may involve medication administration, respiratory support, medical equipment, mobility assistance, treatment schedules, skilled observation, or other clinical responsibilities within the independent nurse’s scope of practice.
TLC coordinates High-Acuity Nursing Care and Nursing Support for Complex Medical Conditions throughout California.
Depending on individual circumstances, independent nurses may support clients living with Spinal Cord Injuries, Progressive Neurological Conditions, ALS, Multiple Sclerosis, and other conditions requiring ongoing skilled nursing care.
For clients requiring respiratory assistance, TLC may coordinate nurses experienced in Ventilator and Tracheostomy Care, subject to professional qualifications, applicable medical orders, and the established plan of care.
These examples illustrate some of the services our registry may help coordinate. We welcome inquiries involving other medical conditions, specialized treatments, and individualized nursing needs.
Nursing Support During Recovery and Healthcare Transitions
Returning home after hospitalization, surgery, or rehabilitation can involve significant adjustments for clients and families.
Even when someone is medically ready for discharge, additional nursing support may be helpful for managing medications, following treatment instructions, monitoring recovery, or maintaining continuity with the established medical team.
TLC coordinates Hospital-to-Home Transition Support and Post-Surgical Private Duty Nursing for individuals who may benefit from extended or continuous nursing during recovery.
Ongoing nursing may also be considered following a serious illness, complex surgery, or Organ Transplant.
Some clients require additional support only during the initial recovery period, while others benefit from Long-Term Recovery Nursing as their condition and needs evolve.
Nursing schedules can be reviewed and adjusted when appropriate, allowing arrangements to reflect changing circumstances.
Supporting Comfort, Independence, and Familiar Routines
For individuals living with ongoing medical needs, private nursing can offer more than assistance with clinical tasks.
Receiving nursing support in familiar surroundings may help clients maintain personal routines, privacy, independence, and meaningful time with loved ones.
Experienced independent nurses can become familiar with a client’s communication style, baseline condition, medications, medical equipment, and established plan of care.
For older adults who wish to remain at home, Aging in Place Nursing Support may be part of a broader plan to maintain comfort and independence.
For individuals living with chronic or progressive conditions, Nursing Support for Chronic Illness Management may help establish consistency in daily care.
Each nursing arrangement should reflect the individual’s clinical requirements, preferences, and personal goals.
Thoughtful Nurse Matching and Continuity of Care
Consistency can be especially valuable when nursing support is needed regularly.
As nurses become familiar with a client’s medical needs, routines, preferences, and established care plan, communication and day-to-day care may become more organized and predictable.
TLC emphasizes Thoughtful Nurse Matching by considering professional qualifications, clinical experience, geographic location, scheduling availability, and individual preferences.
For ongoing arrangements, we work toward recurring schedules with familiar independent nurses whenever possible.
We also recognize that schedules occasionally change because of illness, availability, personal commitments, or evolving care needs.
When adjustments are necessary, our Care Coordination team helps communicate scheduling changes and explore appropriate coverage options.

Nurse Case Management and Clinical Care Coordination
Ongoing nursing arrangements sometimes involve multiple nurses, physicians, specialists, medications, treatment plans, and healthcare appointments.
In these situations, experienced independent Registered Nurses may provide an additional level of professional clinical support through Nurse Case Management & Care Coordination.
Depending on the client’s needs, services may include nursing assessments, individualized care planning, medication reconciliation, communication with treating providers, evaluation of changing clinical needs, and coordination among members of the healthcare team.
Nurse case management may be arranged alongside ongoing private duty nursing or as a separate nursing service.
The independent RN provides professional clinical services within their scope of practice, while TLC coordinates referrals, scheduling, and related administrative communication through our registry.
Planning Ahead for Ongoing Nursing Support
When nursing needs can be anticipated, beginning the conversation early provides additional time to explore appropriate arrangements.
This is particularly helpful when coordinating multiple independent nurses for recurring shifts or continuous coverage.
Families may wish to begin planning before a scheduled surgery, hospital discharge, rehabilitation transition, relocation, or anticipated change in medical condition.
Advance planning provides more opportunity to discuss clinical requirements, scheduling preferences, nurse qualifications, and continuity goals.
Our guide to Advance Planning for Private Nursing Support offers additional information about preparing for care.
We also understand that healthcare needs do not always arise with advance notice.
If circumstances change unexpectedly, we encourage you to contact TLC so our team can review your needs and discuss what may be possible based on independent nurse availability.
What Happens After You Contact TLC Nurse Solutions?
Our goal is to make the coordination process understandable, welcoming, and straightforward.
Understanding Your Needs
We begin by learning about the individual requiring nursing support, their location, medical circumstances, requested schedule, and any important preferences or concerns.
If you’re still exploring how many nursing hours may be appropriate, we can explain the types of arrangements our registry coordinates.
Reviewing Clinical and Scheduling Requirements
Our team reviews the information needed to identify appropriately qualified independent nurses, including clinical considerations, applicable physician orders, scheduling requirements, and anticipated duration of care.
For ongoing or 24-hour arrangements, we also discuss how multiple nursing shifts may be organized.
Identifying Available Independent Nurses
TLC seeks independent RNs or LVNs whose professional qualifications, experience, location, and availability align with the requested services.
Whenever possible, we prioritize arrangements that support familiar nurses and consistent schedules.
Coordinating the Nursing Arrangement
Once the proposed arrangements have been reviewed and agreed upon, TLC helps coordinate scheduling, administrative details, and communication with the relevant parties.
Independent nurses remain responsible for their professional clinical assessments, nursing judgment, and care within their licensed scope of practice.
Ongoing Communication and Support
As care continues, our team remains available to assist with scheduling questions, changes in nursing needs, coordination requests, and administrative matters.
We encourage clients, families, and authorized representatives to keep us informed when circumstances or preferences change.
Communication With Families and Healthcare Professionals
Extended nursing arrangements often involve several people working together to support one individual.
Depending on the circumstances, TLC may communicate with clients, family members, physicians, Case Managers, Discharge Planners, and other authorized healthcare or professional care partners.
For families working with fiduciaries, trustees, or established advisors, we also coordinate nursing arrangements involving Professional Care Teams.
Clear communication helps everyone understand scheduling arrangements, responsibilities, and whom to contact when questions arise.
TLC coordinates nursing referrals and administrative matters through our registry. Independent nurses communicate clinical information and collaborate with authorized healthcare professionals as appropriate to their assignments and professional responsibilities.
Our team respects the importance of privacy, professional boundaries, and each person’s role in the care relationship.
Billing and Payments
For private-duty nursing arrangements established directly with TLC Nurse Solutions, clients receive invoices from TLC and make payments to TLC according to their Client Service Agreements.
Independent nurses submit their billing to TLC. They do not separately invoice clients or collect payments directly for nursing services coordinated through our registry.
TLC coordinates private-pay nursing services and does not bill Medicare, Medi-Cal, or private health insurance companies or submit insurance claims. Clients may independently explore reimbursement through applicable insurance policies or other funding arrangements.

When nursing services are coordinated through a specialty pharmacy, healthcare facility, or another healthcare organization, billing follows TLC’s agreement with that organization. Patients receiving nursing through these arrangements are not responsible for TLC’s organizational billing.
Payment Security
TLC will never contact you by email or text requesting that you change payment instructions or send funds to a different account. If you receive such a request, do not send payment. Call TLC directly at 510-780-1800 to verify the communication.
