Joining the TLC Nurse Solutions registry is designed to feel clear, simple, and supportive. Our three-step process helps you submit the credentials and information needed for review so we can thoughtfully coordinate nursing opportunities aligned with your experience, skills, availability, and areas served.

Step 1: Access Your Forms

Visit the Downloadable Forms section below to access the required onboarding documents. These forms help us maintain a complete credentialing file and prepare for future nursing opportunities.

Step 2: Submit Your Documents

Complete the forms and upload them through the Fill Out and Upload Docs section on this page, along with all required supporting documentation.

• PDF format only
• No photos, screenshots, or document-sharing links
• Most smartphones include document-scanning tools that can create clear PDF copies
• Forms may be completed electronically using Adobe, Google Docs, DocuSign, or your preferred PDF editor
• Legible handwritten forms are accepted when electronic completion is not available

Step 3: Confirmation from TLC

Once your documents are received, TLC Nurse Solutions will review your submission and email you regarding any additional information or next steps needed to complete the onboarding process.

Once your file is complete, you may be contacted about nursing opportunities that align with your experience, availability, geographic preferences, and client and healthcare partner needs.
